Don't mute the console input if we can't read the keyboard
...
This makes sure you can hit Ctrl-C if you don't have permission to access the raw keyboard device.

Add and update include guards
...
Include guards in most changed files were missing, I added them keeping
the same style as other SDL files. In some cases I moved the include
guards around to be the first thing the header has to take advantage of
any possible improvements compiler may have for inclusion guards.

Implemented full evdev keyboard text support
...
This is based on the Linux kernel driver, and has fallback mapping tables in case we aren't connected to a virtual terminal.
